Obviously, can ladies get pregnant when they are in the 30s, in the 40s, in the 50s, in the 60s, so what is the main determinant of ladies conceiving and my answer is going to be about ladies conceiving with their own eggs to start with. The chances of ladies conceiving with their own eggs in their 20s, early 30s is one in four, it drops to one in ten in the 40s, it goes even further lower when the ladies are in the range of 45 to the extent that the chances of a live birth with the ladies own eggs in ladies who are older than 46 or 47 is very unlikely. I repeat, I’m not saying impossible, I’m saying very unlikely and I’m excluding from my discussion ladies who are having treatment with donor eggs because those ladies can get pregnant irrespective of their age bracket and I’m also excluding ladies who have frozen their eggs or their embryos when they were younger because the main determinant why ladies start having lower chance of getting having a child is the chromosomal abnormalities in eggs and obviously egg donation will be bypassing that problem because by definition donor embryos that are created from eggs that were donated by donors who are younger than 35 and that will also be bypassing these who froze their eggs when they were in their 20s or early 30s. So, the odds of ladies conceiving at the age of 50 exist but mainly ladies at the age of 50 will mainly conceive from donor eggs or from embryos that they created when they were younger. Unfortunately there’s no way to know exactly what is the percentage of eggs that are chromosomally normal or not but we know from general genetic studies that we assume approximately one out of four embryos at the blastocyst states to be normal in ladies in their early forties, while that goes down to one in seven to one in eight embryos at the blastocyst stage in ladies at the age of approximately 45 and that is the main reason why ladies will start struggling to conceive with their own eggs above a specific age.

The egg quality is always going according to the age of the patient. Until 40-42 years old we can say that the egg quality can be still not perfect but can be still okay for pregnancy.
Personally and in our Centre we rarely have pregnancies after the age of 42-43 years old – I mean naturally. The oldest pregnancy that I have seen in my life I would say was 44-45 years old. I have not seen it older than that naturally conceived pregnancy but neither by IVF.
The most pregnancies after this age are coming from oocyte donation.
After 44-45 years old till 50 I would say that all pregnancies are from egg donation. What a woman should be aware of is that she should try naturally up to certain age. After 40-41 she could go step further and think about IVF and not lose so much time

That’s a difficult question. It’s not if all of those things are happening, it’s not impossible to get pregnant into your 40s and late 40s. The chances get lower and lower but it’s not impossible. I can say, I think, we’ve published a paper looking at the oldest woman to get pregnant with her own eggs from IVF and this was a frozen embryo but she was just shy of her 48th birthday when she produced this embryo – incredibly unusual though and the chance of getting pregnant from an embryo with your own eggs above 45 is really low – about 1% but people do get pregnant naturally into their late 40s but it’s very unusual.
