If you have absent or persistently irregular cycles, then you need to make sure that you don’t have ovulatory dysfunction. If ovulatory dysfunction is diagnosed and the first steps of improving ovulation do not have any positive effect, then you need to consider having ovulation induction treatment, so that you can predict and schedule the frequency of intercourse which will be around the ovulation day, so that a pregnancy takes place.

Here in primary care, ovulation induction doesn’t happen so, they do come to secondary care for that. So, the process at our center is there, once referred, which could be a GP or could be my colleagues as well if they are seen in different bits of the department and then, they say fertility as their main concern then they are directed to our unit. So, here the first point in appointment is by the specialist nurse so, she goes through a questionnaire, arranges all the necessary preliminary investigations then, they can be seen by any of the three consultants (including me) and we just go through the investigation results and based on the diagnosis, if it is ovulation induction, we carry out here using clomiphene or gonadotropins. If it is tubal surgery needed then, we carry it out here for distillate block or proximal then, we do proximal tubal cannulation, fibroids we do myomectomy here then, for sperm problems we straight away refer them to for IVF ICSI to the Chester Centers, for endometriosis also if they need surgery, we carry out here. So we do a fair bit before the reference.
