Who should proceed with PESA (Percutaneous Epididymal Sperm Aspiration)?

PESA stands for Percutaneous Epididymal Sperm Aspiration and it is a procedure performed for men who are having sperm retrieved for IVF/ICSI. Procedure is done in case of obstructive azoospermia.

PESA is where you put the needle directly into the epididymis and aspirate. The epididymis is where the sperm is stored normally, so if you produce sperm it is stored in the tissue. In most cases you get sperm directly that way.

PESA is Percutaneous Epididymal Sperm Aspiration. Epididymis is a coiled tube at the top of the testicles that stores the sperm, so the sperm normally is stored there for a few days and they mature. Again, as I said, if there is some part of the tube which is missing and the sperm is not coming out through ejaculation due to different reasons, then we can get the sperm from the epididymis which is swollen as there is a lot of sperm there. So we just insert the needle into the epididymis and basically try to aspirate the sperm , If we do this, go into the epididymis it is called Percutaneous Epididymal Sperm Aspiration- PESA, or if we go through the testicle, it is called TESA.

It is a very important procedure that is now getting very popular because with this technique the chance to find spermatozoa is highest. With this technique, the doctor, the urologist need to have a lot of experience – this is a very delicate technique but it is becoming a gold standard now because we use this technique in patients where it is difficult to find spermatozoa. It gives them a chance to retrieve it.
