What is the best treatment for unexplained infertility? Who can consider IUI and who should proceed straight to IVF? What statistically is giving the best outcome with unexplained fertility diagnosis?

In the UK, NICE recommends IVF for unexplained subfertility. IUI has shown pregnancies in unexplained subfertility however, the success rate is far lower. There have been studies done to show the role of IUI in unexplained subfertility however, people have argued whether they have offered the treatment in couples who may have conceived naturally anyway because the duration of the subfertility was not that long but definitely there is a success with IUI in unexplained infertility but the success rate is far lower compared to the standard IVF which is recommended.

Other than continuing to try, because there is always going to be an ongoing chance, the discussion around that is where do you give that label, so many people will get a diagnosis of unexplained subfertility or infertility after a year of trying. In my view, the best treatment then is to try for a little longer unless there are other things that might take you into treatment earlier. But the difficulty is that even complex treatments such as IVF aren’t fool proof, it does not guarantee success so you need to weigh up the balance of what you can achieve over another year of trying on your own which is not insubstantial, 75 percent, 85 percent, 95 percent chance of pregnancy, so there is a good proportion of people who will still conceive on their own without any intervention at all and the complexity of getting into complex treatment. IVF can give couples a very good chance of success and it perhaps bypasses some of the things that we cannot necessarily explain or understand. So for me, IVF, because it gives us diagnostic information, it treats some problems that we can’t otherwise identify and will give couples where they have less of a chance of ongoing cumulative pregnancy, a chance over a short period of time that is enhanced.
